This is the enrolment form for the Lane Leader Multi Squad. It is a term-based program and each term requires a new enrolment.

We are privileged to have the use of the heated and enclosed (winter terms) HMS Bloemhof 25m Pool in Stellenbosch

Per swimmer for 9-week program - R 1780-00

I undertake to participate at own risk for Lane Leader swimming club and will not hold the club or company or its coaches responsible for any loss which may occur as a result of participation in activities of the club. I also understand that I am not able to hold the Club responsible for any costs that may be incurred in relation to Sports injuries. The Club will not take responsibility for any medical costs or enter into negotiations with practitioners in this regard. I hereby confirm that I am aware of, take cognisance and submit myself to the rules, practices constitution and bylaws pertaining to the Lane Leader swimming club as well as the facility rules of the venue concerned. Once a swimmer is enrolled he/she is responsible for the full terms program payment. There are no make up lessons. The full fee is payable irrespective of number of lessons attended.

I do hereby, on behalf of myself, and insofar as the minor child/children reflected above, and on their behalf, acknowledge that:

1. I am aware that Lane Leader Stellenbosch (Pty) Ltd utilise certain facilities at Hoër Meisieskool Bloemhof (“Bloemhof”) at Stellenbosch to render swimming training sessions to said minor child/children.

2. I am aware of the prevailing COVID-19 pandemic and that there has been incidences of contagion in the greater Stellenbosch area.

3. I am aware that even though Bloemhof and its governing body have taken all reasonable measures to prevent COVID-19 infections and spread amongst pupils and staff members and have taken all reasonable steps to sanitise its facilities and keep such facilities free of contamination, instances of contagion may occur at the Bloemhof school and its facilities, and to which me and the above minor child/children may be exposed to during our accessing of the Bloemhof facilities and participating in the Lane Leader Stellenbosch (Pty) Ltd training sessions; And accordingly I do hereby, on behalf of myself and the said minor child/children, waive, abandon, unconditionally, in favour of Bloemhof, its principal and staff members, in both their personal and representative capacities, and the members of the Bloemhof governing body, and absolve and indemnify the aforesaid fully, completely and unconditionally, in respect any and all claims that may arise, of whatever nature, including claims for consequential losses or damages and injury or death, or legal costs, that may arise from me accessing the Bloemhof grounds and facilities and the said minor child/children participating in the swimming lessons of Lane leader Stellenbosch Pty Ltd at the Bloemhof facilities.
